Angelina's brother James was the inspiration for her "Where's Jamie?!" scene.

Angelina avoided any communication with Winona Ryder when making the movie, she claimed that if she saw anything human about Winona she wouldn't be able to act out the sociopath character of Lisa Rowe so effectively.

At the end of the movie, Susanna states that by the 1970's, most of her friends were released. Although the film doesn't elaborate on this, the book reveals that Georgina and Lisa were later released.

The author, Susanna Kaysen, was among the detractors of the film, angered by the "melodramatic drivel" Mangold used with the script by conjuring up many stories that never happened in the book (such as Lisa and Susanna running away together).

Filmed along Main Street in Mechanicsburg, Pennsylvania, as well as in Harrisburg State Hospital in Harrisburg, Pennsylvania.

The hospital Claymoore was based on McLean Hospital, the actual institution in Susanna Kaysen's Memoir.

The gun that Daniel Craig uses in the Tomb of the Dancing Light is a Walther P99 pistol, the signature weapon of James Bond. He later wielded this weapon again as 007 in Casino Royale (2006).
