1
Schindler's List (1993,  R)
Schindler's List
To me, this will always be the most disturbing film ever made.
2
Threads (1984,  Unrated)
Threads
By far one of the most extreme films I've seen, but it's so brilliantly pulled off that it's impossible to not admire it.
3
Ex Drummer (2007,  Unrated)
Ex Drummer
This shocking and sexually explicit gorefest is probably the most offensive film ever made, but it's also one of the funniest films I've ever seen in years!
4
Meet the Feebles (1989,  R)
Meet the Feebles
This is my favorite Peter Jackson film, no doubt. However, it disturbed me so deeply that I will likely never watch it ever again.
5
Begotten (1991,  Unrated)
Begotten
Absolutely moving, but it contains some of the most extreme visions of suffering I have ever seen.
6
Eraserhead (1977,  Unrated)
Eraserhead
I am in love with the world of this film.
7
Jacob's Ladder (1990,  R)
Jacob's Ladder
This film is like watching a person get tortured for two hours, except that it's worse than physical torture. It's mental and psychological torture. It's more depressing and disturbing that most films I've ever seen. It's so much more worse due to the fact that it's so real and has been known to happen.
8
Taxidermia (2006,  Unrated)
Taxidermia
Simply put, one of the most beautiful and most engrossing experiences I've ever had with a film. My favorite film of 2006!
9
Combat Shock (1986,  Unrated)
Combat Shock
By far one of the most depressing and repulsive films I've ever seen, yet it's realism and it's grittiness is so well done and the film itself is very well made.
10
Maniac! (1980,  Unrated)
Maniac!
Gory and unpleasant shockfest. Well-made, but extremely sickening. One of the best sleaze-fests ever put to celluloid.
11
Tras el cristal (In a Glass Cage) (1989,  Unrated)
Tras el cristal (In a Glass Cage)
This film is extremely effective in showing us where cruelty toward another human being can get us. It's extreme, it's shocking, and it's brilliant.
12
Seul Contre Tous (I Stand Alone) (One Against All) (1998,  Unrated)
Seul Contre Tous (I Stand Alone) (One Against All)
Extremely disturbing, horrific, and shocking film, but undeniably brilliant and heart wrenching. Philippe Nahon's performance is unforgettable. This is a pretty solid film, though at times it becomes too abrasive for it's own good. It's still a strong piece of film making.
13
A Clockwork Orange (1971,  R)
A Clockwork Orange
The deconstruction of the human body, soul, and mind has never been so horrifying.
14
The Texas Chainsaw Massacre (1974,  R)
The Texas Chainsaw Massacre
THE TEXAS CHAIN SAW MASSACRE is a masterpiece of mayhem, gore, horror, shock, and insanity. Once you see it you will never forget it.
15
Vulgar (2002,  R)
16
Addio zio Tom (Goodbye Uncle Tom) (1972,  Unrated)
Addio zio Tom (Goodbye Uncle Tom)
On one hand it is well done, but on the other hand it's intentions are somewhat exploitative and disturbing. It's one of the most extreme films I've seen, but appropriately and gleefully so. Do I give it a high star rating for being well made? Or do I give it a low star rating for the same reason? Hmm. I think 2 a 1/2 will work.
17
The Cook, the Thief, His Wife & Her Lover (1989,  R)
The Cook, the Thief, His Wife & Her Lover
A grotesque masterpiece of humor, horror, romance, and politics.
18
Happiness (1998,  Unrated)
Happiness
Hilarious film about pedophilia, rape, masturbation, and death. It's shocking as hell, but it doesn't exploit it's subjects. Very challenging piece of filmmaking with one hell of a cast.
19
À l'intérieur (Inside) (2007,  Unrated)
À l'intérieur (Inside)
THIS film is impossible to forget. It's one of the goriest films I have ever seen, and one of the most horrifying. If you want to see a truly deeply sick, fucked up, shocking, gorefest of a horror film, this is the one to see! Also it's only 80 minutes long, so it's short, sweet, and to the point. It'll make you scream, it'll make you cry, and it will make you cringe.

Sweet Movie (1974,  Unrated)
Sweet Movie
Holy shit was this obscene! I wasn't sure what to make of it! There's a girl that wins a virginity contest(don't ask) who marries a rich man with a solid gold penis. She gets karate chopped by a black bodybuilder and shipped off to a faraway land in which she has sen with a mariachi who she inadvertently gets stuck to. Then she goes to a commune and then the rest of the film consists of people vomiting and urinating on each other and in each other's mouths and it's all for real before they then start defecating while it cuts between old Nazi archive footage of babies getting tortured. There's also a ship where this woman abducts four young boys and then proceeds to sexually molest them. I don't even want to go into detail in what happens next. Very twisted film, but it's definitely well-made.
