It is a nightmarish world of alienation and loss of identity, An absurd man named Henry, overwhelmed with fear and passivity is being haunted by his degenerated self, symbolically manifested in his degenerated infant of gender which cannot be identified. That's just one interpretation among lots of alternatives which try to explain the latent logic of Lynch's subconscious.
